IMMUNIZATION FEE UPDATES

The Ohio Department of Health will no longer be providing low cost vaccines to children with

Private Insurance coverage effective March 2015.  Due to these changes, Wyandot County Health Department is now required to purchase all pediatric vaccines through an outside medical supplier.

 

We are contracted with the following private insurance companies:

Aetna, Anthem, Medical Mutual and United Health Care.

 

If you have a private insurance company we are not contracted with, you are required to pay all fees at time of service. We will provide an itemized receipt for you upon your request at the time of service that you may submit to your insurance on your own for reimbursement.

 

Fees will include:  Vaccine fee(s) and Administration fee (s).

 

Client must provide insurance information at time of immunization appointment scheduling and show proof of insurance at time of clinic appointment.
