What is a Community Health Assessment?

The Wyandot County Community Health Assessment is the result community organizations partnering to improve community wellness and quality of life in Wyandot County.

The health assessment gives a snapshot of our community, as well as a comparison to the state and nation. It is our hope that the data presented in this report provides valuable information for developing strategies, educating and implementing services focused on wellness, access to care, and unmet community needs. It provides additional insight into our health status, and has the potential to play a significant role in influencing the Health Alliance's course of action in supporting health, wellness, and prevention in the community.

Comparing data from prior years will allow community partners and stakeholders to observe trends, write data-driven grants, formulate strategic plans, and to work toward "A Healthier Wyandot County."

The Wyandot County Health Alliance uses this information to strengthen its efforts to bring about healthy changes in the community. We hope this report will be a valuable tool to you as an agency or community member and encourage you to be part of the movement to improve health and wellness.

Upcoming Community Health Assessments

The Wyandot County Health Alliance conducts Community Health Assessments every three years. The 2024 CHA is currently underway. This year surveys will be conducted via SurveyMonkey online. Until 2024, surveys were sent through the mail at random to Wyandot County residents to fill out and return. If you would like to participate in the 2024 CHA survey, please click the button above. All answers remain confidential and anonymous. Once all data is gathered, a draft Community Health Assessment is posted for community input, questions, and observations to finalize the data. The data collected in the CHA is utilized by the health alliance as it creates Community Health Improvement Plans.

Wyandot County Public Health is nationally accredited through the Public Health Accreditation Board (PHAB). Established in 2007, PHAB is the non-profit organization that administers the national accreditation program, which aims to advance and transform public health practice by championing performance improvement, strong infrastructure, and innovation.
