• Issues burial transit permits

• Issues certified copies of birth and death certificates for $25.00

Effective October 16, 2009:

Uncertified copies of birth or death certificates are no longer available.

As part of Amended Substitute House Bill 66 (ORC 3705.242), the price of certified copies of birth and death certificates has increased by $1.50. The fee increase will be used to fund the Family Violence Prevention Fund, which the bill created. The current price for a certified birth or death certificate is $25.
 
 

Birth and Death Certificates

All Wyandot County birth and deaths from 1908 to present are recorded at the Wyandot County Health Department. Births and Deaths from 1867 to 1908 are recorded in the Probate Division of the Wyandot County Courthouse.

Download and Print the form below.  Fill out form completely and bring or mail the form to the Wyandot County Health Department:  127-A South Sandusky Avenue, Upper Sandusky, Ohio,43351.  For births and deaths between 1867 and 1908, please contact the Probate Division at (419-294-2302). Each copy is $25.00.
