Wyandot County
Home Health Agency is a service for residents of Wyandot
County, providing services such as Registered Nurses, Home
Health Aides, Physical Therapists, Occupational Therapists,
Speech Therapists, and Passport Provider.

administered program provided by your Wyandot County Board
of Health

a Title III Grant under the Older Americans Act
administered through the Ohio Department of Aging and the
Ohio District 5 Area Agency on Aging, Inc.
The Home Health
Service provides visits to patients who are confined to home,
are under a physician's care, and are in need of part time or
intermittent care.
Home Health
Services are provided on a charge per visit basis. Payment
sources may be: Medicare, Medicaid, Self Pay, Private
Insurance, United Way, Title 3B or State Block Grant. Services
will not be denied because of inability to pay.
Personal
Donations and Memorials allow Wyandot County Home Health
Agency to provide services regardless of the ability to pay.
Wyandot
County Home Health is nonprofit; approved by Ohio Department
of Health and certified by the Social Security Administration
for participation in Medicare, Medicaid, and other programs.
