Birth and Death Certificates

All Wyandot County birth and deaths from 1908 to present are recorded at the Wyandot County Health Department. Births and Deaths from 1867 to 1908 are recorded in the Probate Division of the Wyandot County Courthouse. 

Obtaining a Birth or Death Certificate
In December 2010, Wyandot County Health District gained computer access to all registered births that occurred in Ohio after 1908. This new technology allows the issuance of a certified birth certificate at your local health department, regardless of county of birth.

To obtain a certificate, download and print the form below.  Fill out the form completely and bring or mail the form to the Wyandot County Health District:  127-A South Sandusky Avenue, Upper Sandusky, Ohio, 43351.  For births and deaths between 1867 and 1908, please contact the Probate Division at (419-294-2302). Each copy is $25.00.

Death/Birth Request Form
